On or about 09/24/12, at the site of 149-29 34th Ave. Flushing, NY Employees working on the ground were in danger of head injury from falling objects such as, but not limited to tools and roof materials from the roof. Protective helmets were not provided. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19

ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

The ladder side rails extended one foot above the upper landing surface. The employees were exposed to the hazard of falling approximately 20 ft to concrete ground below. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
