29 CFR 1926.21(b)(6)(i):  Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required:         On or about September 17, 2012, Employees were instructed to enter into a street basin manhole by removing the manhole cover to enter the storm drain and install stainless steel screens at the drain opening facing the street level.  Employees were not trained on the hazards of entering into confined spaces.
