29 CFR 1926.21(b)(6)(i): Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required: a) At the worksite, on or about 9/25/12: An employee was directed to enter a confined space and break concrete with a pneumatic hammer, without classifying the space and without providing training for this employee on the nature and hazards associated with working in a confined space. b) At the worksite, on or about 9/25/12: An employee was directed to return to the confined space and continue to break concrete with a pneumatic hammer, while this space had not yet been classified and no training had been provided to the employee on the nature and hazards associated with working in a confined space. Abatement certification must be submitted for these items.
