29 CFR 1926.452(k)(4):     Ladders used to support ladder jacks shall be placed, fastened, or equipped with devices to prevent slipping.    1289 Ewell Court, Post Falls:  On or about 25 September 2012 and times prior thereto, employee installing siding was on ladder jack scaffolding that was not fastened or supported to the roof to prevent slipping.  Employee was working approximately 13 feet above the ground.  This condition exposed personnel to a fall hazard.
