The employer provided only Spark Plug type earplugs, which did not provide some employees a suitable fit.     b) On or about 10/11/12, it was determined that the employer had failed to determine and provide an effective means of residual hearing protection for a deaf employee working in the production area through consultation with an ear, nose, and throat specialist (ENT) and audiologist.
