Brunswick, NJ Site Maintenance Area: The lockout program provided did not include, but not limited to: 1/ the program was not documented and tailored to the site specific equipment to be worked on 2/ there were no specific steps documented to de-energize the equipment 3/ means to control the use and identity of locking devices 4/ designated personnel involved in the program, etc. Equipment include: Industrial washers, dryers, HVAC system (air conditioners/heating units), etc. Observed on or about 9/21/2012.

Brunswick, NJ Site Maintenance Area: Where employees are required (on a limited basis) to perform live electrical testing and/or trouble-shooting on machinery/equipment, protectors including but not limited to electrical rated gloves (minimum 250 to 500 volts), insulated tools, etc. were not provided for each electrical worker. Site equipment voltage is 220. Observed on or about 9/21/2012.

Brunswick, NJ Site Throughout Facility - Including but not limited to Resident Hallways, Boiler Room, Dietary, etc. 1/ Doors that lead into adjacent rooms thar are not exits 2/ Doors that have small character marrkings that cannot be seen from a short distance 3/ Doors that have markings on the wall next to them that cannot be seen due to high storage 4/ Doors with the exit arrow pointing in the wrong direction Observed on or about 9/21/2012.
