29 CFR 1910.147(c)(4)(ii):  Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):    a/ CareOne E. Brunswick, NJ Site Maintenance Area:  The lockout program provided did not include, but not limited to:   1/ the program was not documented and tailored to the site specific equipment to be worked on   2/ there were no specific steps documented to de-energize the equipment  3/ means to control the use and identity of locking devices  4/ designated personnel involved in the program, etc.    Equipment include: Industrial washers, dryers, HVAC system (air conditioners/heating units), etc.    Observed on or about 9/21/2012.

29 CFR 1910.37(b)(5):   Door(s) which were not an exit or way of exit access, and which were so located as to be likely to be mistaken for an exit, were neither identified by a sign reading "NOT AN EXIT" or similar designation nor identified by a sign indicating their actual character:    a/ CareOne/E. Brunswick, NJ Site  Throughout Facility - Including but not limited to Resident Hallways, Boiler Room, Dietary, etc.    1/ Doors that lead into adjacent rooms thar are not exits  2/ Doors that have small character marrkings that cannot be seen from a short distance  3/ Doors that have markings on the wall next to them that cannot be seen due to high storage  4/ Doors with the exit arrow pointing in the wrong direction    Observed on or about 9/21/2012.
