29 CFR 1910.101(b):     "Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6.    Located on the outside of the facility at the loading dock area, there were two argon cylinders that were to be used in part for welding that were not secure, thereby exposing the employees to a struck-by hazard if a cylinder was to be knocked over.

29 CFR 1910.178(l)(1)(ii):     Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).    a.  At the facility, employees were required to move materials with a forklift without any forklift training, thereby exposing the employees to the hazards associated with forklift operation.

29 CFR 1910.303(b)(8)(i):     Electric equipment shall be firmly secured to the surface on which it is mounted.  Note to paragraph (b)(8)(i) of this section: Wooden plugs driven into holes in masonry, concrete, plaster, or similar materials are not considered secure means of fastening electric equipment.    a.  Located in the middle of the facility, along a building support column, there were three receptacle boxes connected together that were not secured to its surface, thereby exposing the employees to an electrical shock hazard.

29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    a.  At the workplace, there were exposed wires protruding from a junction box that was without a cover located on a wall near the loading dock, thereby exposing the employees to electrical shock hazards.
