Location: 420 East Main Street, Building #3, Unit #17, Branford, CT 06405    The Werner 16 foot fiberglass extension ladder, model D6216-2, type IA was placed atop the scaffold platform of the 6 foot 1 3/4 inch long, 29 inch wide and 5 foot 11 inch high tubular welded frame mobile scaffold.    On September 21, 2012, an employee working on the ladder sustain a fall resulting in serious bodily injuries when the ladder became displaced while being set upon the scaffold platform.
