29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck was dismounted and within 25 feet of the truck still in his view, the load engaging means were not fully lowered, controls neutralized, and the brakes set to prevent movement: a/ Inside of the warehouse, the operator of a Toyota Fork Truck, m/n TOY-LP, s/n 7FGCU45-70559, dismounted this material handling machine and walked away to retrieve a sticker. The load engaging means were not fully lowered. The elevated load of steel was approximately three feet above the concrete floor and weighed approximately 5067 pounds.
