29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: (a) Garden Valley, Idaho; Sixshooter thinning project, on or about September 25, 2012, and at times prior thereto, several employees were exposed to eight-hour time-weighted averages of 89-91 decibels (dBA). Note: Abatement certification is required for this item.

(b) Garden Valley, Idaho; Sixshooter Project, on or about September 25, 2012, and at times prior thereto, first aid kit had one and not two large guaze pads. (c) Garden Valley, Idaho; Sixshooter Project, on or about September 25, 2012, and at times prior thereto, first aid kit did not have elastic wraps. (d) Garden Valley, Idaho; Sixshooter Project, on or about September 25, 2012, and at times prior thereto, first aid kit did not have splints. (e) Garden Valley, Idaho; Sixshooter Project, on or about September 25, 2012, and at times prior thereto, first aid kit did not have emergency directions.
