The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a) 8330 State Road, Philadelphia, PA- Employer does not have a written lock-out/tag-out program for service and maintenance of equipment.   On or about 9/21/12    Note: Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.36(g)(2):   An exit access was not at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access.    a) 8330 State Road, Philadelphia, PA-Supply Warehouse-The exit access was obstructed with merchandise stored near the exit discharge.  The exit door was measured at approximately 36 inches, while the exit route was reduced to 27 inches wide.     Note: No abatement certification or documentation required.
