29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. (a) Southeast Corner of Building - On or about September 11, 2012 eight employees were exposed to multiple fall and puncture hazards while working. There were several piles of boards with nails protruding from them. (b) Main Doorway on West Side of Building - On or about September 11, 2012 eight employees were exposed to multiple fall hazards while working. There was a large pile of wooden boards lying on the ground at the main entrance to the building. (c) Northwest Corner of Building - On or about September 11, 2012 eight employees were exposed to multiple fall and puncture hazards while working. There was a pile of several boards with nails protruding from them.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. (a) South Side of Building - On or about September 11, 2012 one employee was exposed to a falling object hazard by standing directly underneath another employee that was using a nail gun. The employee on the lower level was not wearing a hardhat to protect him from falling tools and equipment.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide. (a) South Side of Building - On or about September 11, 2012 two employees were exposed to a fall hazard by working on a wooden fabricated scaffold with a walkway that was only 6 inches wide.
