29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification. 3743 W. Tayjan, Post Falls, ID 83854; on or about 26 September 2012 and times prior thereto, a 120 volt, 4-plug junction box located on the second floor and a junction box located on a post next to the sidewalk were not used in accordance with the listing , labeling and or certification. A 120 volt, 4-plug junction box was wired to an extension cord. This condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. 3743 W. Tayjan, Post Falls, ID 83854; on or about 26 September 2012 and times prior thereto, an orange extension cord was not connected to devices and fittings so that strain relief is provided. This condition exposed employees to an electrical shock and electrical burn hazards.
