29 CFR 1926.304(i)(1):     Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:    3743 W. Tayjan, Post Falls, ID 83854; on or about 26 September 2012 and times prior thereto, employees were using a 10 inch Ryobi table saw, serial number XX093451698, model BTS16, to cut siding. The blade guards were removed. This condition exposed employees to amputation and struck by hazards.

29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation.    3743 W. Tayjan, Post Falls, ID 83854; on or about 26 September 2012 and times prior thereto, employees assembled an aluminum Werner Pump Jack  and a  frame supported  scaffold without installing mud sills or on a stable surface.  The scaffolds were being utilized and employees were observed on the scaffold. This condition exposed employees to a fall hazard of up to 28 feet.

Tayjan, Post Falls, ID 83854; on or about 26 September 2012 and times prior thereto, employees assembled an aluminum Werner Pump Jack supported scaffold without ensuring the scaffold supports and planking were plumb as required by the manufacturer.  The scaffolds were being utilized and employees were observed on the scaffold. This condition exposed employees to a fall hazard of up to 28 feet.

For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.      3743 W. Tayjan, Post Falls, ID 83854; on or about 26 September 2012 and times prior thereto, Employees erected an aluminum Werner Pump Jack Scaffold without the use of toeboards, netting or similar method to prevent objects from falling on employees working below. Tayjan, Post Falls, ID 83854; on or about 26 September 2012 and times prior thereto, employer did not ensure that the path to ground from circuits, equipment, or enclosures was permanent and continuous. Two instances were observed. This condition exposed employees to an electrical shock hazard.            (a) A 10 inch, 120V 15A, Ryobi table saw, serial number XX093451698, model BTS16 was missing the ground plug.             (b) A 7  inch, 120V 13A Skilsaw Circular (Worm Drive) saw, serial number H4862, model HD77 was missing the ground plug.
