29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Industrial Screen Process Services, Inc., located at 17 17th Street, Toledo, OH 43604: On or about September 24, 2012, the employer failed to establish a lockout / tagout program for the control of the hazardous energy, which includes procedures, training, and periodic inspections, while performing activities such as, but not limited to, changing saw blades and drill bits on machines such as, but not limited to, Startrite Band Saw (Model V900F, Serial # 202657), Enco Mill Machine (Model YL-90L-4, No Serial #), and 12-Speed Industrial Drill Press (Manufactured by Orbit Machine Tools, Model OR-1458, Serial # 86202). It created a potential for the machine to be activated while maintenance is being performed on the machine, exposing employees to crushing hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.176(b):     Storage of material created a hazard:    Industrial Screen Process Services, Inc., located at 17 17th Street, Toledo, OH 43604: On or about September 24, 2012, the employer did not ensure that shelving unit in the facilitys fabrication shop, used to store chemical products such as, but not limited to, Huntsman RenCast 3266 Epoxy (RP 3266 Resin) and Huntsman Ren 3266 Amine (RP 3266 Hardener), was secured in a manner to prevent any products from falling over due to unstable structure of the unit. It exposed employees in the area to struck-by hazards from products on the shelves and the shelving unit itself.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    Industrial Screen Process Services, Inc., located at 17 17th Street, Toledo, OH 43604: On or about September 24, 2012, the employer failed to ensure that 12-Speed Industrial Drill Press (Manufactured by Orbit Machine Tools, Model OR-1458, Serial # 86202), located in the facilitys fabrication shop, was properly guarded in order to prevent employees from inadvertently being caught by the rotating chuck during the drill press operation.    No additional abatement information is needed for this item.

Employees are required to use chemical products such as, but not limited to, Huntsman RenCast 3266 Epoxy (RP 3266 Resin) and Huntsman Ren 3266 Amine (RP 3266 Hardener), which were stored in the facilitys fabrication shop.
