29 CFR 1910.272(j)(2)(ii):  The employer shall immediately remove any fugitive grain dust accumulations whenever they exceed 1/8 inch (.32 cm) at priority housekeeping areas, pursuant to the housekeeping program, or shall demonstrate and assure, through the development and implementation of the housekeeping program, that equivalent protection is provided:    a.  Inside the back manlift, fugitive grain dust accumulations of 1.75 to 4 inches which had covered the floor were not immediately removed.    b.  In the back manlift house on the top floor, fugitive grain dust accumulations of 0.25 to 1.5 inches which had covered surfaces such as the floor, top shelf of the cabinet, and bottom shelf of the cabinet were not immediately removed.    c.  In the back manlift area on the first floor, fugitive grain dust accumulations of 0.25 to 4 inches which had covered surfaces such as the floor, breaker box, individual breakers, conduit for the breaker box, horn, and conduit for the horn were not immediately removed.    d.  In the lower basement, fugitive grain dust accumulations of 0.134 to greater than 12 inches which had covered surfaces such as the floor, stairs, chute for small leg, boot for small leg, board laying across boot for small leg, chutes on East side of small leg, boot for big leg, boards on top of block structure, wall light, and receptacle for sump pump were not immediately removed.

Equipment, wiring methods, and installations of equipment in hazardous (classified) locations shall be intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:    a.  In the back manlift house on the top floor, electrical equipment including, but not limited to, the duplex receptacle and extension cord plugged into the receptacle in a Class II, Division 1 location were not intrinsically safe, approved for a Class II, Division 1 location, or safe for a Class II, Division 1 location.    b.  In the back manlift area on the first floor, electrical equipment including, but not limited to, the circuit breaker box in a Class II, Division 1 location was not intrinsically safe, approved for a Class II, Division 1 location, or safe for a Class II, Division 1 location.    c. At the entrance to the back manlift area on the first floor, the employer did not have a combustible dust hazard sign where explosive grain dust atmospheres may occur due to the handling of grain.    b.  At the entrance to the dump floor and basement, the employer did not have a combustible dust hazard sign where explosive grain dust atmospheres may occur due to the handling of grain.    c.  At the back entrance to the basement, the employer did not have a combustible dust hazard sign where explosive grain dust atmospheres may occur due to the handling of grain.

Chemical-specific information must always be available through labels and safety data sheets:    a.
