29 CFR 1910.147(c)(4)(ii)(D):   The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures:  (a)  Farmers Grain & Feed machine specific lockout procedures did not address the requirement to verify proper de-energization exposing employees to potential restart of equipment due to inadequate lockout.

29 CFR 1910.147(c)(6)(i):   The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  (a)  The employer had not accomplished a periodic inspection of the lockout / tagout procedures.  Accomplishing a review of lockout tagout procedures ensures continuity of procedures and proper application.  Inadequate review of changes to equipment and machinery can lead to improper lockout.

29 CFR 1910.334(a)(2)(i):   Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  (a) An extension cord used to vacuum the pile of soybeans had the outer jacket pulled away and was missing the grounding pin.  This exposed employees to electrical shock hazards.
