29 CFR 1910.305(g)(2)(iii):     Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:      a)  215 County Avenue, On Muller Martini Saddle Stitch Machine #6 in the Area of the Omron Limit Switch, Secaucus, NJ.    Bindery operator making an adjustment in the machine was exposed to electric shock as one of the flexible power cords connecting to the limit switch had tension at the connection point with wires being exposed.                                                  Violation observed on or about 09/05/2012.
