29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Lakepark Industries, Inc., located at 40 Seminary St., Greenwich, OH 44837: On or about September 26, 2012, the employer failed to properly train employees to lockout/tagout OHMA Vertical Pneumatic Press # 0806013, aka Press 120 (Model # PS832-1430-CFF-.50-UR8-32, S/N 8127) located in the operations area. Employees were conducting activities such as, but not limited to, changing dyes on the press without locking and tagging it out.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(7)(i):     The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    a)Lakepark Industries, Inc., located at 40 Seminary St., Greenwich, OH 44837: On or about September 26, 2012, the employer failed to properly train employees to lockout/tagout OHMA Vertical Pneumatic Press # 0806013, aka Press 120 (Model # PS832-1430-CFF-.50-UR8-32, S/N 8127) located in the operations area. Employees were conducting activities such as, but not limited to, changing dyes on the press without locking and tagging it out.    b) Lakepark Industries, Inc., located at 40 Seminary St., Greenwich, OH  44837:  On or about September 17, 2012, the employer did not ensure employees who operated the OHMA Vertical Pneumatic Press # 0806015, located in the operations area, were aware of the hazards associated with the malfunctioning of the press cycle, resulting in an employee placing their hand into the point of operation to remove a part when the machine failed to cycle properly.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

A.	Lakepark Industries, Inc., located at 40 Seminary St., Greenwich, OH 44837: On or about September 26, 2012, the employer did not provide the required injury or illness description on the OSHA 300 Log for the calendar year 2012. The descriptions in Column F, case numbers 2, lacked the required information including, but not limited to, the injury or illness and the parts of the body affected.    B.	Lakepark Industries, Inc., located at 40 Seminary St., Greenwich, OH 44837: On or about September 26, 2012, the employer did not provide the required injury or illness description on the OSHA 300 Log for the calendar year 2012. The descriptions in Column F, case numbers 3, lacked the required information including, but not limited to, the injury or illness and the parts of the body affected.    No additional abatement information is needed for this item.
