29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):    On or about January 30, 2013 and times prior thereto, at this facility, the employer did not provide a fit test to employees required to wear a 3M negative pressure respirator when spray painting and sanding.  Employees were issued negative pressure respirators for working with hazardous chemicals such as, but not limited to IND100 Acrylic Lacquer Thinner and 2.8 VOC Shop Primer.

29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer:    On or about January 30, 2013, at this facility, employees required to wear respirators wore dirty and unsanitary 3M respirators while performing sanding and spray painting operations. The employer did not ensure respirators were cleaned and disinfected after each use to prevent worker exposure to chemical hazards.
