(a) On or about 09/27/2012 at 2614 South Clinton Avenue, South Plainfield, New Jersey 07080 CSHO observed employees of Prestige Construction Services working at the top edge of trench/excavations when the vertically installed rebar at the bottom of the trench/excavations were not capped resulting in the employees being exposed to impalement hazards.
