29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a) On September 21, 2012; front roof area; employees were replacing the shingles on a 10 in 12 pitch roof without proper fall protection in place exposing them to a 25 foot fall.

29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, or a barricade was not used to keep the activities or traffic away from the ladder. On or about September 21, 2012; northside of home; ladder used to access the roof area was not secured to prevent from displacing while in use, exposing employees to 20 foot fall.
