29 CFR 1910.110(b)(13)(ii)(b)(7): Release of fuel when any section of piping or appliances was disconnected was not minimized by either using an approved automatic quick-closing coupling or closing the valve nearest to the appliance and allowing the appliance to operate until the fuel in the line is consumed. Worksite: The employer did not ensure that there were no open flames or other potential sources of ignition present in areas where Liquefied Petroleum Gas (LPG) created a possible hazardous atmosphere. On or about 9/10/12, two employees were seriously injured from an explosion that occurred while performing work in a hazardous atmosphere, following an uncontrolled release of LPG into the workplace.
