29 CFR 1910.147(c)(4)(i):     Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.     a. At the workplace, the employer did not ensure energy control procedures were utilized when employees were exposed to the unexpected energization, startup or release of stored energy that could occur and cause injury.  On or about June 6, 2012 an employee had a finger amputated on the 9738 control arm assembly machine.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a. At the workplace, the employer did not protect employees from an amputation hazard created by a pinch point between a hydraulic cylinder and a reinforcement bracket. On or about June 6, 2012 an employee had a partial finger amputated on the 9738 control arm assembly machine.

29 CFR 1904.7(b)(3):     How do I record a work-related injury or illness that results in days away from work? When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known.    a.  Located at Zhongding USA, Inc. in Stratsburg, Ohio:    On the 2012 OSHA 300 Log the employer did not provide the number of days the injured or ill worker was away from work as required in Column k.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.    Located at Zhongding USA, Inc. in Stratsburg, Ohio:    a.  On the 2012 OSHA 300 Log the employer did not provide all information required in Column F such as the actual injury, the cause of injury and the body part affected.
