29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Gibbsboro site - On or about 10/01/12 employees, without head protection, were exposed to head injury when working from different heights of a 13 foot high x 28 foot long x 5 foot deep fabricated frame scaffolding.     NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    a) Gibbsboro, NJ construction site - An employee was working from an approximate 6 foot high x 35 foot long x 5 foot deep fabricated frame scaffold that was missing two base plates, on or about 10/01/12.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a) Gibbsboro, NJ construction site, West end of building - Employees climbed the end frames and cross braces when accessing the approximate 11 foot 4 inch level masons platform and the approximately 13 foot high laborers platform of the approximately 13 feet high x 28 feet long x 5 foot deep fabricated frame scaffold, on or about 10/01/12;    b) Gibbsboro, NJ construction site, South side of building - An employee climbed the end frames and cross braces to access the approximate 6 foot 6 inch high fabricated frame scaffold, on or about 10/01/12.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a)  Gibbsboro, NJ construction site - Employees working between approximately 11 feet 4 inches and 13 feet did not recognized the various hazards associated with the incorrect setup of the approximate 13 foot high x 28 foot long x 5-7 foot deep fabricated frame scaffold they were working from; hazards included: no fall protection, no proper means of accessing the work platforms, no head protection, missing base plates, setup out of plumb, on or about 10/01/12.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
