On September 27, 2012 and times prior to, employees were using powered industrial vehicles without receiving adequate training such as but not limited to practical training and practical exercises performed by the trainee, exposing employees to stuck-by hazards while operating powered industrial trucks.

29 CFR 1910.178(l)(3)(i)(M): Any other operating instructions, warnings, or precautions listed in the operator's manual for the types of vehicle that the employee was being trained to operate. On September 27, 2012, and times prior to, employees were inadequately trained to use powered industrial due to not following the operators manual. This condition exposed employees to potential electrical hazards while operating the truck where overhead energized power lines were present.
