29 CFR 1910.217(c)(3)(iii)(f): Mechanical power press(es) using presence sensing point of operation devices were not provided with guards to protect all areas of entry to the point of operation not protected by the presence sensing device(s): The adjustable barrier guard on V & O 11-ton part-revolution power press #4 did not protect areas of entry to the point of operation on the right side of the press. On or about September 25, 2012, an employee sustained an amputation injury while reaching around the light curtain, through an open adjustable barrier guard, into the point of operation.

29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment: Production Floor: At the time of the OSHA inspection on or about October 2, 2012, the employer had not established a monthly inspection schedule for the company's six V & O 11-ton part-revolution power presses.
