29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    (a) Jobsite: 3946 Windgap Avenue, Pittsburgh, PA 15204: On or before October 1, 2012, an employee was working unprotected and exposed to fall hazards ranging from approximately 28 feet 5 inches to 29 feet to ground level while on a 6/12 pitched roof. The employee was removing shingles from the roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) Jobsite: 3946 Windgap Avenue, Pittsburgh, PA 15204: On or before October 1, 2012, the employer did not train their employee on the hazards associated with falling while removing shingles on a steep sloped roof and what fall protection methods shall be used. An employee was anticipating that a 12 inch debris stop constructed from plywood would prevent a fall.
