All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a) On or about September 26, 2012 electrical disconnect was not shut off to disable power on the car wash towing conveyer system while performing maintenance. b) On or about August 22, 2012 electrical disconnect was not shut off to disable power in the sluge pit to prevent water and/or material from entering the pit while clening it out.
