29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b):     A.)  TE Construction, 363 Brusselles Street, St Marys PA  15857, main roofs of the residence, on or about October 1, 2012 and times prior thereto:  Employees performing roofing work were not utilizing a mean or methods of fall protection when exposed to a fall hazard. Fall heights ranged from 8-ft up to 18.3 feet.    B.)  TE Construction, 363 Brusselles Street, St Marys PA  15857, front porch roof, on or about October 1, 2012 and times prior thereto:  Employees performing roofing work were not utilizing a mean or methods of fall protection when exposed to a 10 feet fall hazard.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    A.)  TE Construction, 363 Brusselles Street, St Marys PA  15857, front of the residence, on or about October 1, 2012 and times prior thereto:  Employees were utilizing a Werner FE1020-2 fiberglass extension ladder to gain access to the porch roof.  The ladder was only extended one foot above the porch roof.    B.)  TE Construction, 363 Brusselles Street, St Marys PA  15857, front of the residence, on or about October 1, 2012 and times prior thereto:  Employees were utilizing a Werner fiberglass folding ladder in the folded position to gain access from the porch roof to the main roof of the house.  The ladders side rails extended less than 1-foot above the roof line.

TE Construction, 363 Brusselles Street, St Marys PA  15857, front of the residence, on or about October 1, 2012 and times prior thereto:  An employee on-site was removing shingles and nails from the porch roof of the structure.  The employee was not utilzing safety eyewear. Worker was wearing prescription eyewear that was not designed and manufactured to be utilized as safety eye wear.     B.)  TE Construction, 363 Brusselles Street, St Marys PA  15857, front of the residence, on or about October 1, 2012 and times prior thereto:  An employee on the lower porch roof was not wearing head protection when an employee over was removing roof jack backets.
