TE Construction, 363 Brusselles Street, St Marys PA 15857, front of the residence, on or about October 1, 2012 and times prior thereto: Employees were utilizing a Werner FE1020-2 fiberglass extension ladder to gain access to the porch roof. The ladder was only extended one foot above the porch roof. B.) TE Construction, 363 Brusselles Street, St Marys PA 15857, front of the residence, on or about October 1, 2012 and times prior thereto: Employees were utilizing a Werner fiberglass folding ladder in the folded position to gain access from the porch roof to the main roof of the house. The ladders side rails extended less than 1-foot above the roof line.

TE Construction, 363 Brusselles Street, St Marys PA 15857, front of the residence, on or about October 1, 2012 and times prior thereto: An employee on the lower porch roof was not wearing head protection when an employee over was removing roof jack backets.
