29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Basement, 161 Endicott Street, Boston: Workers were not informed of the hazards of carbon monoxide, a combustion product and the avoidance and unsafe condition of using gasoline powered tools in a confned area. On 9-28-12, during a drain pipe installation project, two workers were overexposed to carbon monoxide while cutting concrete flooring with a gasoline powered saw in a confined basement work area. 29 CFR 1926.55(b): Feasible administrative or engineering controls were not implemented to reduce employee exposure(s): Basement 161 Endicott Street, Boston: On 9-28-12, administrative and/or engineering controls were not implemented to reduce airborne concentrations of carbon monoxide below the Threshold Limit value of 50 ppm, while workers were operating a gasoline powered saw to cut into concrete flooring. Controls may include but are not limited to: 1. Use of local exhaust ventilation in the work space while equipment capable of emitting carbon monoxide was operated. 2. Install carbon monoxide detectors in the work space to ensure workers are warned when levels exceed 50 ppm. 3. Substitute gasoline with electrically powered equipment.
