29 CFR 1915.55(a)(7): An unsuitable truck, chain or other steadying device was used to keep cylinders from being knocked over while in use. a) A propane tank was not secured while in use on pier 27 next to the tugboat repair. b) Portable welding tanks- located at Pier 27 were not properly secured and could fall.

29 CFR 1915.73(e): When employees were working near the unguarded edges of decks of vessels afloat, they were not protected by personal flotation devices meeting the requirements of 29 CFR 1915.158(a). Employees were standing on rubber tires attached to the pier working on a barge without wearing personal floatation devices.
