29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:    a) The Ottawa Grain & Feed Co. - Ottawa, Ohio: On or about October 2, 2012, the employer failed to keep the floor of the roller mill basement free of accumulation of dust. Laboratory analysis of the dust indicated that it was explosive, exposing employees to hazards from fire and explosion.

Employees were exposed to hazards from chemicals such as, but not limited to, propylene glycol and beta-Cyfluthrin.

Employees were exposed to hazards from chemicals such as, but not limited to, propylene glycol and beta-Cyfluthrin which were present in the insecticide that was used inside grain bins.
