29 CFR 1910.272(g)(1)(ii):  Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by equally effective means or methods:      a.) The Ottawa Grain and Feed Co. Inc., located at 206 S. Oak St. Ottawa, OH; On or about October 2, 2012, the employer did not assure the sweep auger was deenergized before entering the bottom portion of the Small 20 bin (Small Silo) to begin the clean out. Employees were exposed to amputation hazards while inside the bin with one pendant to control the auger.

29 CFR 1910.272(g)(3):     In the grain handling facility an observer, equipped to provide assistance, was not stationed outside the bin, silo, or tank being entered by an employee:    a.) The Ottawa Grain and Feed Co. Inc., located at 206 S. Oak St. Ottawa, OH; On or about October 2, 2012, the employer did not assure an observer was stationed outside the Small 20 Bin (Small Silo) while employees entered to complete a clean out. Employees were exposed to amputation hazards while using the sweep auger and performing clean out procedures in the bottom section of the bin.
