29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: Construction site: On or about October 2, 2012, Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: 1) Two extension cords used by employees were not examined prior to being used by employees. One extension cord had its ground pin removed and both extension cords showed visible signs of damage to the outer insulation. 2) The Skil HD5687-01 7-1/4-in Skilsaw extension cord used by employees had visible damage to the outer insulation.
