29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Residential rooftop on or about October 2, 2012, employees were installing asphalt roof shingles on a 9-in-12 pitch roof (steep roof) without fall protection such as, but not limited to, guardrails, toe-boards, safety net system or personal fall arrest system (PFAS), thus exposing the employees to a fall hazard up to 12 feet. b) Residential rooftop at the edge of the gable roof on or about October 2, 2012, employees were installing asphalt roof shingles on a 9-in-12 pitch roof (steep roof) without fall protection such as, but not limited to, guardrails, toe-boards, safety net system or personal fall arrest system (PFAS), thus exposing the employees to a fall hazard up to 23 feet. ABATEMENT CIRTIFICATION IS NOT REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Front elevation on or about October 2, 2012, employees used an aluminum extension ladder to traverse the roof eave using an unsecured extension ladder. The extension ladder was positioned at the roof eave and did not extend 3-feet beyond the roof edge, thus exposing employees to a fall hazard up to 12-feet. ABATEMENT CERTIFICATION IS NOT REQUIRED
