Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts: (a) On or about October 2, 2012, for the employees working around and under the Challenger 2-post above ground lifts, Model 27000, with a 9,000 lbs. capacity, where the lifts have broken telescoping stops and missing contact pads at Iverson Chrysler Center, located at 600 South Burr Street in Mitchell, South Dakota. Abatement Note: Among other methods, one feasible and acceptable method of abatement would be to follow the requirements of ANSI/ALI ALOIM and/or ANSI B153.1 which includes, among other requirements, the following: 1. Institute a preventive maintenance program according to the manufacturer's specifications. 2. Provide training to employees on proper use and care of the lifts. 3. Ensure all lifts are rated and marked with their load capacity. 4. Schedule and complete all periodic and annual inspections.
