Location: Store    On or about 10/1/12, the employer did not keep the following but not limited to areas clean and in a sanitary condition;    1. Men's stock room (hanger room) - visible mold was observed along the back wall and insulation by the ceiling.  2. Loading dock desk - visible mold was observed along the desks back wall and along the desks shelf by the radio.

29 CFR 1910.22(a)(3):     Every floor, working place, and passageway were not kept free from protruding nails, splinters, holes, or loose boards:    Location: Store    On or about 10/1/12, employees were exposed to trip and fall hazards from the lifting parquet floor and rugs placed over the damaged floor in the following areas but not limited to;    1. Lower level, Childrens Department  six different areas of lifted flooring  2. Lower level, Womens Department  fifteen different areas of lifted flooring with carpets, signage, and cones in the aisle ways.  3. Lower level, Coat Department  three different areas of lifted flooring with carpets and signage.  4. Lower level, Elevator  lifted flooring with carpets overlapping one another  5. Upper Level Elevator  Several observations of uneven floor surfaces and areas where parquet floor was taped down.
