29 CFR 1910.217(b)(4)(i): Pedal mechanism(s) on mechanical power press(es) full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals:    The employer does not ensure the pedal mechanism on the Johnson Mechanical Power Press labeled #11 is protected to prevent unintended operation. This violation was observed on or about October 2, 2012, in the Shop area where employees were exposed to struck-by and caught-in hazards when operating the Johnson Machine Press without protection to prevent unintended operation from falling or moving objects or by accidental stepping onto the machine's pedal operational mechanism.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the pedal mechanisms on machines are protected to prevent unintended operation.

29 CFR 1910.217(b)(6)(i): Two hand trip(s) on mechanical power press(es) using full revolution clutches did not have the individual operator's hand controls protected against unintentional operation:    The employer does not ensure that the two hand trip hand controls are protected against unintentional operation on the Bliss 60 ton mechanical power press labeled Number 1. This violation was observed on or about October 2, 2012, in the Shop area where employees were exposed to struck-by and caught-in hazards when operating the Bliss 60 Ton Press without protection to prevent unintended operation from falling or moving objects or by accidental stepping onto the machine's pedal operational mechanism.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the two hand trip mechanisms on machines are protected to prevent unintended operation.

29 CFR 1910.217(b)(6)(ii): Two-hand trip system(s) on mechanical power press(es) using full revolution clutches did not incorporate an anti-repeat feature:  The employer does not ensure that machines equipped with a two-hand trip system incorporate an anti-repeat feature.  a. This violation was observed on or about October 02, 2012, in the shop area where employees were exposed to struck-by and caught-in hazards when operating machine number 1, a Bliss 60 Ton mechanical power press.  The machine was equipped with a two-hand trip system but has no antirepeat feature.  b. This violation was observed on or about October 02, 2012, in the shop area where employees were exposed to struck-by and caught-in hazards when operating a machine number 3, a Niagara E-75mechanical power press.  The machine was equipped with a two-hand trip system but has no antirepeat feature.   c. This violation was observed on or about October 02, 2012, in the shop area where employees were exposed to struck-by and caught-in hazards when operating a machine number 5, a Stamtec mechanical power press.  The machine was equipped with a two-hand trip system but has no antirepeat feature.   d. This violation was observed on or about October 02, 2012, in the shop area where employees were exposed to struck-by and caught-in hazards when operating machine number 7, a Niagara E-110 mechanical power press.  The machine was equipped with a two-hand trip system but has no antirepeat feature.   e. This violation was observed on or about October 02, 2012, in the shop area where employees were exposed to struck-by and caught-in hazards when operating machine number 9, a Komatsu 45 ton mechanical power press.  The machine was equipped with a two-hand trip system but has no antirepeat feature.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that two-hand trip systems on machines using full revolution clutches incorporate an antirepeat feature.
