(a) Quality Roofing, 451 Boardwalk Street, Fort Collins, CO 80525; On and before October 2, 2012, the exposing employer failed to ensure that 4 employees performing roofing work were provided with adequate fall protective measures. Condition exposed employees to fall hazards of approximately 20 feet.
