29 CFR 1910.334(a)(2)(iii): Attachment plug connected to a receptacle (including an on a cord set), the relationship of the plug and receptacle contacts were not visually checked to ensure that they were of proper mating configurations. Oil Waste Reclamation Building - Electrical plug on starter equipment had a missing ground plug and was plugged into a grounded extension cord.
