Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. a) On or about August 29, 2012, and at times prior, employees were exposed to electrical shock because a power strip used in the plating area was connected to other extension cords or portable power outlets instead of directly to a permanently installed branch circuit receptacle.

29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose. a) On or about August 29, 2012, and at times prior, employees were exposed to electrical shock because wire splices for a solenoid in the tin plating area which were joined by wire nuts were not insulated nor contained in an enclosure or junction box to protect them from contact.

29 CFR 1910.305(g)(1)(iv): Flexibles cords and cables were used for purposes prohibited by subparagraphs (a) through (e) of this paragraph: a) On or about August 29, 2012, and at times prior, employees were exposed to electrical shock and fire hazards because flexible cords were used as a substitute for fixed wiring (conduit) along the walls in the preclean and plating areas. In particular, flexible wiring was hardwired into a light switch which controlled a fan along the wall by the preclean tanks. b) On or about August 29, 2012, and at times prior, employees were exposed to electrical shock and fire hazards because flexible cords were attached to the building structure and surfaces such as pipe in the tin/lead plating area.

A pinch point existed between chemical barrels and a box which reduced the access width to 19 inches.
