29 CFR 1926.404(f)(7)(iv):       Under conditions described in paragraph (f)(7)(iv)(C) of this section, exposed noncurrent-carrying metal parts of cord and plug-connected equipment which may become energized, was not grounded:      a) On or about 10/2/12, on construction site at the GM Tonawanda Powertrain Plant # 5, Buffalo, NY: A Ridgid Model 300 Compact Pipe Threader located in Area "150 LGE Cranks Install 2" was used to thread & cut pipes. The Pipe Threader was connected to 120-volt ac power via a series of two extension cords, the first of which was plugged into a permanent receptacle inside the building. The plug on the cord of the Pipe Threader lacked a grounding prong.      NO ABATEMENT CERTIFICATION REQUIRED
