General-duty citation text
29 CFR 1910.178(l)(3)(i)(M): Powered industrial truck operators did not receive initial training on truck-related topics including operating instructions, warnings, or precautions listed in the operators manual for the types of vehicles that the employees were being trained to operate: a) Forklift activity at the jobsite on or about and at times prior to October 3, 2012, workers were observed being repositioned while elevated, using the platforms mid-rail and top-rail to achieve greater height, and the forklift operator did not remain behind the controls of a SKY TRACK forklift while employees were elevated. The employer did not ensure forklift training content was presented and evaluated to ensure it outlined the safe operating procedures as outlined for a SKYTRACK, rough terrain forklift, S/N 0160003806, in that it was not specific to the manufacturers instructions and warnings that pertained to the hazards of traveling with personnel elevated, requirement to remain at the controls, and the requirement for personnel working from the platform to never use the railing to achieve additional reach or height, thus exposing employees to struck-by and fall hazards. ABATEMENT CERTICATION IS NOT REQUIRED
