29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On or about 10/03/2012 in the maintenance shop and electrical panel "E" opening were not closed, exposing employees to electrical hazards. a) On the Clausing C225W band saw, the indicator lights and started botton were missing exposing employees to an electrical hazard. b) Inside Panel "E" circuit breakers were removed and knock outs were not used to cover the openings. Employees were exposed to an electrical hazard when accessing the panel to turn the lights in the facility on or off.
