29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. (a) Jobsite - 327 2nd Street, Freeport, PA: On or about October 3, 2012, employees were exposed to fall hazards approximately 70 feet to the river below from a hole that was not protected in the bridge. The hole measured approximately nine feet, nine inches in length by three feet, three inches in width.

29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. (a) Jobsite - 327 2nd Street, Freeport, PA: On or about October 3, 2012, an employee was observed using a Milwaukee hand grinder to grind metal strips without wearing sleeves on his arms to protect his skin from the sparks that were being produced while he was grinding the metal. (b) Jobsite - 327 2nd Street, Freeport, PA: On or about October 3, 2012, an employee was observed without protective clothing on his arms while he was arc welding to install and expansion on the bridge.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: (a) Jobsite - 327 2nd Street, Freeport, PA: On or about October 3, 2012, employees were using an extension cord that was powering their chipper gun. The insulation to the chipper gun was stripped away and exposed the electrical wires.
