29 CFR 1910.22(b)(1):     Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:    At this site, passageways not kept clear and in good repair posed hazards:    a.  at the east side of the covered dock area, a snagging hazard was posed by a sharp unused mount bracket that was left in the passagway along the storage truck/dock edge side at the torso height;    b.  at the back storage yard walking area, a tripping hazard existed from a damaged corner in the concrete slab that was neither patched nor flagged.

29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    At this site, employees were exposed to fall hazards:    a.  at the loading dock bays, temporary guardrails for sections when not in use, at 4 feet or greater;    b.  above the entryway between the offices and covered docks, no mid rail chain was provided for the upper deck storage and no guardrails nor toeboards were provided above the stairway to the upstairs storage area.

29 CFR 1910.151(c):     Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    At the work area between the covered deck and the storage tank yard, the emergency eye wash station was not functional and the shower handle was seized up from lack of regular testing and maintenance.  Employees on a daily basis handle injurious chemicals including but not limited to, acetone, xylene, hydrochloric acid, and caustic.

29 CFR 1910.27(c)(4):     The distance from the centerline of rungs, cleats, or steps to the nearest permanent of the ladder was less than 7 inches:    At the center of the Tank Farm, the employer was using Werner D-1332-2 aluminum portable ladder as a fixed ladder.  The centerline of the tie-on ladder was less than the minimum of 7 inches from the outer wall of the storage tank.
