29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: At this site, passageways not kept clear and in good repair posed hazards: a. at the east side of the covered dock area, a snagging hazard was posed by a sharp unused mount bracket that was left in the passagway along the storage truck/dock edge side at the torso height; b. at the back storage yard walking area, a tripping hazard existed from a damaged corner in the concrete slab that was neither patched nor flagged.

The centerline of the tie-on ladder was less than the minimum of 7 inches from the outer wall of the storage tank.
