29 CFR 1926.451(c)(1):   Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.    a) 1201 Latona St., Phila., PA , North & East Sides, Tubular Welded Frame Scaffolds - Stucco workers were exposed to fall hazards of up to approximately 39 feet while working from the supported scaffold that was not restrained from tipping.   Along the east and north sides of the structure the scaffolds were seven sections wide (for a total of 49 feet)  by six frames high ( for a total of 39 feet) and 5 feet deep.  No guying, tying, bracing, or equivalent means were used to prevent the tipping hazard of the scaffold.  Observed 7/26/2012    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     a) 1201 Latona St., Phila., PA, North & East Sides, Tubular Welded Frame Scaffolds - Stucco workers were exposed to fall hazards of up to approximately 39 feet while working from the supported scaffold platforms that did not extend fully between the front & rear uprights.  None of the working levels were fully planked.  On many active levels there were only 3-4 planks out of a possible seven.  No personal fall arrest systems or safety nets were being utilized.  Observed 7/26/2012    Superior Plastering Inc. was previously cited for a serious violation of this Occupational Safety and Health Standard, which was contained in OSHA Inspection: #314228347, Citation #1, Item #2B and affirmed as a final order on 11/8/2010, with respect to a workplace located at 1417-1425 N. 15th St., Philadelphia, PA.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) 1201 Latona St., Phila., PA, North & East Sides, Tubular Welded Frame Scaffolds - Stucco workers were exposed to fall hazards of up to approximately 39 feet while working from the supported scaffolds that had both missing and deficient guardrails.  Some of the working levels did not have any guardrails.  Additionally, some working levels had crossbracing acting as a mid-rail without a top-rail installed.  Observed 7/26/2012.
