29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about October 3, 2012, at 136 Doyle Street. Toccoa, Georgia: An employee repairing a roof leak was exposed to a fall hazard of approximately 14.8-feet when he used a Werner 24-foot aluminum extension ladder to access and egress the roof that was not extended three feet above the roof landing.

29 CFR 1926.1060(a)(1): The employer did not ensure that each employee was trained by a competent person which would enable each employee to recognize hazards related to ladders and stairs, and the procedures to follow to minimize those hazards. On or about October 3, 2012, at 136 Doyle Street, Toccoa, Georgia: An employee repairing a roof was exposed to a fall hazard of approximately 14.8-feet when the ladder he used to access the roof was not set three feet above the landing. The employer did not provide ladder safety training to the employee for the employee to recognize and minimize the hazards associated with ladder use.
