29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) North side at mid-point of the structure, third floor level; On or about October 2, 2012 and at times prior thereto, an employee installing floor joists blocking was walking on/working from the top of the exterior and interior walls without a means of fall protection.     b) North side at east end of the structure, third floor level; On or about October 2, 2012 and at times prior thereto, an employee installing floor joists blocking was walking on/working from the top of the exterior and interior walls without a means of fall protection.     c) West end of structure, both north and south side, third floor level; On or about October 2, 2012 and at times prior thereto, three employees were laying decking up to the immediate exterior edge without a means of fall protection. 29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Southeast corner of the structure, second floor level; On or about October 2, 2012 and at times prior thereto, after attempting to get down from the top of a 9 foot high interior wall by climbing down the framing, an employee was only provided with a 6 foot fiberglass step ladder as a means for getting down.
